Download a blank activity consent form from the Forms tab of this website, fill in the activity name and date at the top, then at the bottom fill in the name and contact information for an adult leader on the trip, and also fill in the name and contact information for someone not on the trip who in an emergency parents can call if unable to reach you (perhaps your spouse or the scoutmaster). Send this partially completed activity consent form as an attachment to your email to the troop announcing the outing or activity. Everyone on the outing or activity, including adults and siblings, needs to turn in a signed activity consent form.
