give to the leader of the outing

Scoutmaster Conference - Board of Review Form

required for rank advancement
fill out & have it signed by the SPL or ASPL
bring to scoutmaster conference
bring to board of review
turn in to scoutmaster after passing board of review

BSA Youth Application (fillable)

use to join the troop
print and sign both copies
turn in originals (no email) to the recruitment coordinator or the Troop Committee chair

BSA Adult Application (fillable)

use to register with BSA to be able to lead outings or join the Troop Committee
print and sign the background check authorization and all four copies of the application (the fourth copy is to be kept by applicant)
turn in originals (no email) to the Troop Committee chair

Zelle Reimbursement Form (fillable)

use this form for refunds and reimbursements with Zelle if it is for $499 or less

Merit Badge Counselor Application (fillable)

use to become a merit badge counselor
turn in to the merit badge coordinator or the Troop Committee chair

Medical Form (fillable)

Parts A and B (pages 1-3) must be filled out annually by all scouts and registered adults. Parents must sign the forms and attach a copy of both sides of insurance card. Turn in to the troop's medical records coordinator.
Part C (page 4) is required for scouts and adults attending summer camp & any other outings longer than 72 hrs. Must be signed by a physician, physician assistant or nurse practitioner. It is valid for 1 year.
